About the Plan

UNM offers Accidental Death and Dismemberment (AD&D) insurance to active benefits eligible employees through The Hartford. AD&D insurance pays you or your designated beneficiary a sum of money in the case of your accidental death or dismemberment.

Dismemberment can mean loss of eyesight, hearing, speech, or a bodily limb due to an accident.

  • Coverage levels are offered in units of $10,000 increments up to $600,000.

Accidental Death and Dismemberment (AD&D) is a voluntary benefit and is 100% employee paid.

When and How to Enroll

First, verify your eligibility.

Next, you may take advantage of these three enrollment opportunities:

During Open Enrollment, complete the Open Enrollment Life, Disability, and AD&D Web Form available through LoboWeb to enroll or make changes.

Enroll or make changes by completing The Hartford Enrollment Form.

Submit your completed Form to Benefits & Employee Wellness via Secure Document Upload or fax to 505-277-2278 within 60 Calendar Days of your newly benefits-eligible position or your Qualifying Change in Status Event.

Note: If you separate from employment, termination of coverage will occur.

Choosing or Changing Beneficiaries

If you enroll in UNM’s AD&D insurance, there is an important step to ensure you’re making the most of this important coverage – choosing or updating a beneficiary. A beneficiary is the person who will receive the proceeds of your AD&D insurance should you pass away due to an accidental injury. You can choose anyone as your beneficiary and change your designation at any time. And when you add or change a beneficiary online, your designation becomes effective immediately.

Examples of when to consider your beneficiary designation(s) are marriage, birth or adoption of a child, or divorce.

Is Your Benefit Coverage Ending?

If your AD&D coverage is ending and you would like to continue coverage under Portability and Conversion with The Hartford, you must return the completed Notice of Continuation form to The Hartford within 31 days after your group coverage terminates, or 15 days from the date the employer signs the form, whichever is later. (This period may be limited to 91 days after group coverage terminates.)
